If you have ever taken a B complex and wondered whether it was actually doing anything, you are not alone. Not all B complex supplements are created equal, and the difference between one that supports your energy and one that passes straight through often comes down to what is in it and in what form. Here is what the research says.

What Are B vitamins?

B-vitamins are water-soluble vitamins that support metabolism, the nervous system, the immune system, skin, and hair. As coenzymes, B vitamins are essential for a wide range of enzymatic processes, including energy metabolism, brain function, and cellular repair. In contrast to fat-soluble vitamins, which the body can store, it is especially important to get enough B-vitamins from nutrition or supplements on a daily basis, because excess is excreted in urine.

How Different B Vitamin Shortfalls Show Up Differently

One of the lesser-known aspects of B vitamins is that deficiency does not always present in the same way. Different B vitamins affect different systems, so symptoms can vary depending on which vitamin is low. The following gives an overview of what the research associates with low levels of each:

  • B1 (Thiamine): mild deficiencies have been associated with general fatigue, irritability, and disturbed sleep.
  • B2 (Riboflavin): associated with physical weakness and fatigue.
  • B3 (Niacin): low levels have been associated with dermatitis, depression, and anxiety.
  • B5 (Pantothenic acid): associated with dermatitis and diarrhea.
  • B6 (Pyridoxal, pyridoxamine, pyridoxine): deficiency symptoms are primarily neurological, including irritability, impaired alertness, and depression.
  • B7 (Biotin): A low biotin level has been associated with seborrheic eczematous rash.
  • B9 (Folate): A low folate level has been associated with anaemia.
  • B12 (Cobalamins): perhaps the most recognised, with deficiency associated with fatigue and anaemia.

B vitamin shortfalls do not just affect physical energy. They have an impact on how the brain functions, which matters particularly for people with high cognitive and physical demands. The brain accounts for just 2% of body weight but consumes over 20% of the body's total energy. The brain has a dedicated system to actively import B vitamins across the blood-brain barrier, and once inside, it keeps them at concentrations higher than anywhere else in the body. For example, the folate levels in the brain may be up to four times those found in the plasma, whereas biotin levels can be up to 50 times higher in the brain than in the plasma.

Who May Be Running Low Without Knowing It

It is a common assumption that people in developed countries with access to varied food are unlikely to be deficient in vitamins. The research indicates that sub-optimal B vitamin levels are widespread, even among people who eat reasonably well. Several factors can drive this:

  • Modern diets: diets high in processed foods and refined carbohydrates tend to be low in micronutrients, including B vitamins.
  • Vegan diet: B12 is found almost exclusively in animal products, making supplementation important for those following a vegan or plant-based diet.
  • Regular intense exercise: vigorous physical activity has been associated with increased depletion of B vitamins.
  • Malabsorption conditions: conditions such as coeliac disease, Crohn's disease, or atrophic gastritis may impair the absorption of B vitamins.
  • Alcohol: regular or heavy alcohol consumption has been associated with depleted levels of several B vitamins, both through reduced dietary intake and impaired absorption.
  • Medications: certain medications, such as oral contraceptives and metformin, may reduce the absorption of some B vitamins.

What to Look for When Choosing a B Complex

Given the above, here is what is worth paying attention to on a label:

  • All eight B vitamins: a complete B complex should include B1, B2, B3, B5, B6, B7, B9, and B12. Formulas containing only a subset can leave gaps.
  • Active forms where possible: for example, look for methylfolate (B9) and methylcobalamin (B12) on the label.
  • Meaningful doses: government-recommended daily amounts represent the minimum to prevent deficiency, but some research suggests that optimal levels for brain and energy support may be higher.
  • Vegan-friendly if needed: B12 is naturally derived from animal sources, so check the label.
  • Reputable manufacturer.

For many people, a varied and balanced diet can provide adequate amounts of most B vitamins. Supplementation is worth considering when dietary intake is limited, absorption is impaired, or demand is increased.

Overdosing on Vitamin B

Overdosing on vitamin B is rare, since vitamin B is a water-soluble vitamin. This means that excess vitamin B is excreted in urine. However, getting excess vitamin B3 or B6 can cause  adverse impacts, such as stomach problems like nausea or diarrhea, from excessive consumption. In practice water-soluble means that the vitamin is not being stored in the body like fat-soluble vitamin D, and excess is removed with urine from the body. Therefore, getting enough vitamin B from daily food or supplements is important.
